St. Kate鈥檚 strives to receive and address employee complaints, concerns, and reported incidents as soon as practicable and with respect to all parties involved. University policies with specific complaints, concerns, and incident processes are included in the 亚色影库 , and at My St. Kate's under University Policies.

False reporting and retaliation are serious violations of the . We expect that all reports will be made in good faith. If an individual knowingly submits a false report or takes retaliatory action towards a reporter, witness, or other party involved, that individual is subject to disciplinary action by the University as outlined in our campus policies.

 

IMPORTANT ANNOUCEMENT

During the Fall 2022 semester, St. Kate鈥檚 will be implementing a series of new processes to more efficiently respond to employee and student reports. Specifically, on September 12th, the University will update reporting forms to reflect these new processes.  Before this date, students may still use the existing forms linked below.

While we are confident that any concern submitted here will reach the appropriate on-campus resources, you may encounter a brief delay as forms transition. Thank you in advance for your understanding.

How to Report

The purpose of the reporting center is to provide employees with an efficient avenue in which to report complaints, concerns and incidents that may arise during employment.

Report a Concern about a Student

The purpose of this form is to share any non-academic (or overlapping) concerns for a 亚色影库 student. This form is reviewed and managed by staff in the Office of Student Affairs. If you have questions or concerns, please contact Student Affairs at studentaffairs@stkate.edu or by calling 651-690-6778. The Office of Student Affairs is located in Derham 305.

If your concern is primarily academic, please use to submit this concern or refer a student to campus resources.

If you have an emergent concern for any individual, please contact Public Safety at 651-690-8888.

Report a Concern or Complaint

If you do not know which of the reports or forms are appropriate for your situation, you are encouraged to contact the Human Resources office for clarification and guidance. Human Resources can be reached at hr@stkate.edu or by calling 651-690-6565. The Human Resources Office is located in Derham 101 & 113.

 

Report Conduct or Policy Violation

Community members are encouraged to use this form to submit information regarding alleged violations of . This information will be reviewed by Student Affairs staff within two business days of report submission. If you have questions about the contents of this form, please contact the Office of Student Affairs to report any incident that may violate any of the University鈥檚 policies or otherwise impact a student鈥檚 experience at St. Kate鈥檚.

Report Academic Dishonesty

Academic Dishonesty is a violation of university policy and interferes with the educational mission of the institution. Incidents of academic dishonesty will be investigated and resolved according to the process outlined in the academic catalog. Report Bias, Harassment, Discrimination, General Harassment and Bullying

As part of that commitment, the University is committed to a work and educational environment that is free from harassment, discrimination, and bullying.

What is a Bias Incident?

A bias incident is an act of bigotry, harassment, or intimidation that are not crimes, violence, threats or property damage that is motivated in whole or in part by bias based on an individual鈥檚 or group鈥檚 actual or perceived race, color, creed, religion, national origin, gender, age, marital status, disability, public assistance status, veteran status, sexual orientation, gender identity, or gender expression. Bias often stems from fear, misunderstanding, hatred, or stereotypes and may be intentional or unintentional.

Examples of Bias Incidents

Bias incidents can be verbal, physical, or visual. This language and behavior often contribute to or create an unsafe or unwelcoming environment. Some examples include:

  • Name-calling; using a racial, ethnic, or other slur to identify someone; or using degrading language.
  • Creating racist or derogatory images/drawings.
  • Imitating someone with a disability or imitating someone鈥檚 cultural norm or practice.

All employees, students, and other University community members are encouraged to use the following form to report incidents of bias, discriminatory, harassing or bullying behaviors that may be a potential violation of the .

Your report will initiate the beginning of a fact-finding process to gather additional information and to determine the nature of any investigation that will be conducted by the University.

Please note that the University鈥檚 ability to respond effectively may be limited by the level of information provided and/or the anonymity of the complainant. The Bias Incident Response team meets every two weeks to review reports. A Bias Incident Response team member will reach out to follow up.

Report Sexual Assault, Sexual Harassment, and Sexual Misconduct

亚色影库 seeks to foster and maintain a community of mutual respect and concern for all of its members. There can be no greater violation of the terms of that community, or of the essential dignity of any member of it, than an act of sexual assault, sexual harassment, or other forms of sexual misconduct described in this . These acts constitute the deepest affront to University standards and will not be tolerated in any form.

The purpose of the form is to report Sexual Assault, Sexual Harassment, and Sexual Misconduct, all Title IX related incidents. We are required to provide an anonymous reporting form which is met by having this online form. For the victim/survivor completing this form, it is your choice whether to remain anonymous. However, please know that doing so may limit the University's ability to address and assist you.

The Title IX Coordinator and Deputy Coordinators are notified when reports are made via this form. Typically, the details of the form submission with be reviewed no later than the next business day, and a response to the submitter shortly thereafter.
